Thrombophilia is a medical term used to describe the condition where the blood has an increased tendency to clot. There are many reasons why the blood can have this increased tendency. Thrombophilia is usually categorized into two types--acquired and inherited. In acquired thrombophilia the abnormal clotting is usually related to a specific cause, such as prolonged periods of bed rest after surgery, trauma to the leg, or having cancer. People with inherited thrombophilia tend to form clots due to a genetic predisposition inherited from their parents. People with inherited thrombophilia may have a family history of relatives with abnormal or excessive blood clotting. NATT’s prestigious Medical and Scientific Advisory Board (MASAB) met in San Francisco on December 7 to discuss projects for 2009, led by Jack Ansell MD, MASAB Chair. MASAB voted to recommend expansion of NATT’s outreach to patients with atrial fibrillation (a fib). Up to now, a fib patients were not considered part of NATT’s constituency because a fib-related clotting occurs in the arteries, not the veins. However, there is an overriding commonality with venous clots and our current patient constituency, as all face the challenge of life with anticoagulants, an important consideration for the NATT Board when it weighs the merits of MASAB’s recommendation.

The results of this study are highly interesting. Statins have a number of effects other than lowering lipids and this may explain the positive effect seen in this trial on the rate of venous thromboembolism. However, before deducting anything clinical from this trial it is important to keep in mind that this was a primary prevention trial in a healthy population, not a secondary prevention trial, i.e. the trial did not investigate whether statins decrease the risk of (a) venous thromboembolism (VTE) in people at risk for VTE or (b) people who had already had one VTE. Thus, no comments are possible on (a) whether statins are an alternative to traditional DVT prophylaxis (heparin, low molecular heparins, Fondaparinux, warfarin, and the new oral anticoagulants) for people undergoing surgery or being hospitalized, and (b) whether they have any positive effect in people with a previous VTE and, thus whether they might supplement or be alternatives to warfarin.

Antithrombin (AT) is a potent inactivator of thrombin and factor Xa and the major inhibitor of blood coagulation. Inherited AT deficiencies are uncommon, with prevalences in the general population between 1 in 500 and 1 in 5000. They are either quantitative (type I) or qualitative (type II). Type II is subdivided into the more common, but less thrombogenic, type IIb deficiency caused by a defect in the heparin-binding region of AT and the less common, but more thrombophilic, type IIa variant caused by mutations in the thrombin-binding site. A pleiotropic type IIc deficiency also exists. In the evaluation of a thrombophilic individual, a functional AT assay (AT activity) should be used and the diagnosis of AT deficiency only established after acquired causes have been ruled out and repeat AT testing on an additional sample has been performed. A subsequent antigenic AT assay result leads to differentiation between type I and type II deficiency. Further specialized tests help subclassify the type II deficiencies, but this is typically not carried out for clinical purposes, even though it might be helpful to assess thrombosis risk. AT deficiency is associated with an increased risk for venous thromboembolism (VTE) and pregnancy loss.

David Bock was seriously injured when he slipped and fell in Upland, California on March 8, 2008, while visiting there from his home in Arizona. While most people who develop a blood clot in their vein have a deep vein thrombosis (DVT) or a blood clot in their lung (pulmonary embolism), a very small number of people who develop blood clots have a more serious and often life-threatening syndrome called Thrombotic Storm. People with Thrombic Storm experience more than one blood clot in a short period of time. These clots occur in different and sometimes unusual locations in the body.
